Set up reoccurring credit card payments

If you would like to set up reoccurring credit card payments, please download this agreement, and return it to us via Fax (248-726-7015), US Mail (Lightning Bolt Technologies, PO Box 82141, Rochester MI 48308), or via email (Billing@LBTech.com), the form must be e-signed or scanned with a signature if sending it via email.

You will need this username and password every time you wish to log-in to view your invoice. Username: Password: This will stop paper invoices from being mailed to you, however you may always view and print your invoice out from your online billing area.
